Angel Tree Application Form
If your facility is registered with Prison Fellowship for Angel Tree, program applications are included in the chaplain kit that will be mailed to your institution.
When completing your order, please select your facility from the dropdown menu.
- If your facility does not appear in the list, you will need to complete Angel Tree Prison registration before proceeding.

INSTRUCTIONS
- Sign, date, and complete both sides of this form.
- Include a heartfelt message for your children.
- Separate forms are necessary for more than three children and/or more than one parent or guardian. Children are eligible for one gift per incarcerated parent.
- Contact your chaplain for your facility’s deadline. All applications must be postmarked by Oct. 1, however your chaplain may request them at an earlier date.
- Return form(s) to chaplain or mail to the P.O. box provided below.
- Let your family know an Angel Tree® volunteer will be contacting them.
ELIGIBILITY
- I am the parent, step-parent, or someone who acts as a parent to each child listed.
- There are no court orders, restraining orders, or other legal restrictions that stop me from contacting these children or their guardian.
- Each child is 18 years old or younger and lives in the United States.
- I give permission to Prison Fellowship Ministries® and their ministry partners to use this information for the Angel Tree programs and ministries.*
SIGNATURE REQUIRED
*By sharing the information on this form, I give Prison Fellowship Ministries permission to use this information in connection with its Angel Tree ministry and year-round programs, including camping opportunities, and to share any such information (in whatever way[s] and format Prison Fellowship chooses) to any others, such as churches, camps, and/or other ministries, and individual volunteers, who may be participating in or assisting any Angel Tree programs. Please sign on the line below to provide approval and agreement that you meet requirements to participate in Angel Tree. By signing below, I agree that I have read and understand the statements above. Forms without a signature will not be accepted.
